    made, but in vain. The neck of tbe child was within easy reach, and décapitation was tkerefore carried out by means of long scissors. Tbe operation was comparatively easy, as was also tbe delivery of tbe trunk depending on it. The principal dilBculty, kowever, remained in tbe delivery of tbe liead left beliind in the utérus, and tliis difficulty was increased by tbe circumstance tbat in tbe décapitation tbe first cervical vertebra had been left attached to tbe liead, and as a conséquence the foramen magnum could not be made use of. Tbe next thing to be doue was to steady tbe kead, and for tbis purpose a hook was passed into tbe mouth on wbicb traction was made. Tbe assistant, kowever, pulled too liard on tbe book and broke tbe lower jaw. As tbe best point to lay bold of for fixing tbe bead was lost, it was wbile continually rolling about laid bold of by a sharp book carried tkrough tbe roof of tbe orbit and tbe cephalo- tribe applied, tbe assistant at tbe same time affording support to tbe bead by external pressure. Tliis attempt itself was not successful. Tbe ceplialotribe had only seized a small segment of tbe bead and began to slip off, just as the liook lost its bold in tbe same way as in tbe attempt to fix tbe liead by tbe mouth. A similar failure resulted from tbe same process in tbe other orbit ; and as ail means of delivering tbe bead hitherto attempted bad failed, Herr Hegar tried to reach tbe posterior side of tbe bead by carrying bis band up along tbe sacrum, and be was able to get at a point in tbe squamous suture wbicb admitted of tbe introduction of tbe scissors-shaped perforator. Tbis passed upwards along squamous portion of tbe temporal boue, yet its application to tbe squamous suture was very difficult because tbe squamous part overrid tbe edge of tbe pariétal boue, bevelled off in tbe opposite direction, and slipped awav over it. Only after much trouble did tbe instrument pass through tbe suture, and at first it slipped upwards between tbe inner side of tbe skull and tbe dura mater, because its bandle could not be sufficiently depressed. At last tbe membranes were perforated and tbe cranioclast applied ; tbe extraction of tbe bead was tben easily accomplisbed. Tlic placenta followed spontaneously. Tbe patient lay sick for eigbt weeks, feeling extremely weak, witb higli fever and profuse perspirations. Tbe locliial discbarge was moderate, and there were occasional pains in tbe abdomen.
